Medical Records

All patient medical records at Ashe Memorial Hospital are handled by the Medical Records Department. This department ensures that records are kept accurately and consistently.

If you are a patient at Ashe Memorial Hospital, the physicians you see during your stay will send any relevant records to your primary care physician or specialist outside of the hospital. Our providers will consult with your other doctors throughout the course of your treatment.

Your medical record is a confidential document; therefore, you must complete a release form before your record can be released.
