Ashe Memorial Hospital is pleased to present its
Community Health Needs Assessment (CHNA) for 2023. The Board of Trustees approved the 2023 CHNA on December 1, 2023.
The purpose of the CHNA is to assess the needs of the community the Hospital serves. Ashe Memorial Hospital worked alongside a number of individuals and organizations, including AppHealthCare, in the development and implementation of the community health assessment plans.
The process for assessing and addressing the status of health in the county was divided into two phases. The first phase of this process included the collection and analysis of community input through one on one interviews and the accumulation of the most current and available secondary data about the community that the Hospital serves. This phase was completed in November 2023.The second phase will be conducted between January‐May, 2024 through the development of Community Health Action Plans.
Ashe County was ranked #18 among the 100 counties in North Carolina for overall health outcomes in the County Health Rankings.

Recognition
Ashe Memorial Hospital has been fortunate to earn a number of national awards and recognition during our 80-year history.
The Joint Commission on Accreditation of Healthcare Organizations regularly evaluates the operations of and care provided by Ashe Memorial Hospital to ensure our facility continues to meet JCAHO's high standards for certification. JCAHO works with professionals from across the private medical field to define and maintain the standards of quality care. Ashe Memorial Hospital is proud to consistently exceed JCAHO's quality standards.
In 2018, iVantage Health Analytics recognized Ashe Memorial Hospital as one of the Top 100 Critical Access Hospitals nationwide. This designation was awarded after iVantage considered a long list of factors including patient outcomes, patient costs, and ability to communicate information both with patients and externally.
The National Rural Health Association previously recognized Ashe Memorial as its National Rural Health Organization of the Year. The hospital was recognized for serving and improving the health of local residents. Ashe Memorial continues to work toward reducing rates of chronic illnesses, including heart disease and diabetes.
The American Hospital Association gave Ashe Memorial its Nova Award for involvement in the Ashe County Health Council Healthy Carolinians Task Force. This award honors collaborative programs effective in improving community health.
The American Diabetes Association recognized Ashe Memorial Hospital for its efforts to reduce the number of diabetes cases in our community through the Diabetes Self-Management Program. Better nutrition and increased physical activity can help guard against the onset of diabetes, and can reduce the severity of symptoms.
The hospital has also won the North Carolina Prevention Award for Coalition Excellence from Prevention Partners, an organization that recognizes the groups and individuals working to make North Carolina healthier by encouraging lifestyle changes that prevent illnesses.
Ashe Memorial's advanced technology has led to its place among the country's Most Wired Hospitals. Hospitals and Health Networks puts together the annual list to call attention to hospitals effectively using technology to communicate both within the hospital and with patients.
