Ashe Memorial Hospital Transitional Care Services

Ashe Memorial Hospital is committed to delivering the highest quality medical care available. As a Critical Access Hospital, we are able to provide Transitional Care Services to patients who no longer require acute medical care, but may need additional time and care for strengthening and recuperation before they go home.

The short term rehabilitation program is designed to help patients improve functional mobility and self-care independence while also providing additional treatments such as IV antibiotics or wound care. Our transitional care services program is ideal for patients needing short term medical management, orthopedic rehabilitation, physical rehabilitation due to prolonged illness and those recovering from medically complex or multiple traumatic issues, among other cases.

Our team of skilled professionals will work alongside the patient to determine the best treatment plan for his or her own individual needs, providing rehabilitation and 24-hour nursing care from the comfort of Ashe Memorial Hospital as opposed to going to a skilled nursing facility.
